Procedure

1. Download the registration application:
   • Encapsulated PostScript (EPS) Format
   • Word RTF Format

2. Print the application on one sheet of paper. Complete it.

3. Submit application in one of three ways:

Deadlines

To qualify for early discounted rates, registration forms must be faxed or postmarked by October 10, 1996. The last date to advance register by fax or mail is November 7, 1996. After this date you may register at the conference. If applying to participate in workshops or tutorials, please see their specific deadlines.

Payment Information

Registration forms must be accompanied by full payment in order to be processed. Incorrect credit card numbers and declined credit are considered non payments and registration will not take place.

• Checks must be in U.S. Dollars and payable to ACM/CSCW 96.

• Purchase orders, government vouchers, wire transfers, and telephone orders are not accepted.

• Conference and tutorial fees are as follows. (All amounts are U. S. Dollars.)

• Member fees only apply to members of ACM, SIGCHI, SIGOIS, GB/ACM, and GB/SIGCHI.

• Students must submit a copy of their current ID card, or a letter from their department confirming their full-time status at the time of registration.

• Full-day tutorials are 2 units; others are 1 unit. Note: The cost for Tutorial #1 is $30 regardless of membership category or registration date. To register for this tutorial, please follow the instructions on the tutorial's webpage (http://www.massnetworks.org/~nicoley/TutorialAnnounce.html).

• Workshop fees are $30 for a Saturday half-day workshop (W1 or W2), and $50 for a Sunday full-day workshop (W3 through W11).

• One copy of the Conference Proceedings is included in the conference fee. Additional copies of the Proceedings are $55 each.

• Likewise, additional Reception tickets are $50 each.

Refunds

Cancellations made in writing and faxed or postmarked by November 7, 1996 will be accepted subject to a $50 administrative fee. No refund requests will be accepted after that date. "No shows" are not refundable and are liable for the full registration fee.

If you cannot attend, you may send a substitute person. The origianl registrant must submit a written authorization for such a change.

Confirmation

Please allow up to 10 days for mailed confirmation of your registration. We will not fax or email confirmations.
