CHI '95 Conference Registration Form

Conference Dates:

May 6 - 7, 1995: Research Symposium
May 7 - 8, 1995: Workshops
May 6 - 8, 1995: Tutorials
May 9 - 11, 1995: Conference

Registration Deadlines:

March 31, 1995: Forms must be postmarked by this date to qualify for the early registration discount.

April 21, 1995: Forms accepted by Registration Office until this date.

Conference registration will be available at the Convention Center, but it is recommended that you register as early as possible by mailing or faxing both sides of this form along with payment in US dollars to the address indicated below.

Payment:

Registration forms must be accompanied by full payment in US dollars. Forms without payment will be returned to sender. Payment may be remitted by credit card, certified check, or money order. All checks should be made payable to ACM/CHI '95. Purchase orders and government vouchers will not be accepted. Registrations may be submitted by fax or postal mail only. Telephone and e-mail registrations will not be accepted. Faxed forms must be paid by credit card.

Refund Requests:

Refund requests must be submitted in writing and must be postmarked before April 21, 1995. Requests for refunds should be directed to the registration address as indicated below. A 10% cancellation fee will be deducted to cover processing costs. Refund requests received after April 21, 1995, will not be accepted.

Registration Transfers:

If you cannot attend, your registration may be transferred by giving a colleague a letter authorizing the transfer. The letter must be presented at registration.
